PROCEDURE
  1. Client/Applicant will submit application together with the complete requirements to Record Section/Office of the Regional Director
  2. Record Section will:

    • Prepare CAV Form
    • Forward CAV document/s to RD’s Office for approval and signature of RD
  3. Regional Director’s Office will:
    • Approve and Forward the document/s to Records Section
  4. Records Section will:

Correction of Name/Date of Birth Application
REQUIREMENTS
  • Letter request of the student/parents addressed to the Regional Director, DepED ROVII, Sudlon, Lahug, Cebu City
  • Endorsement from the School Registrar of School Principal of the School
  • Copies of Birth Certificate duly certified by authorized Officers
  • Affidavit of (Self/Mother/Father/Guardian) attesting veracity of the child’s data
  • Affidavit of two disinterested persons
  • School Records bearing the wrong name (Diploma, Form 137, Transcript of Records, etc.)
  • ID Picture (2×2) latest
  • Alien Certification of Registration (ACR), if non-Filipino
  • A receipt from Cash Section for documentary stamp
PROCEDURE
  1. Client/Applicant will submit application for request of Correction of Name/Date of Birth to Record Section/Office of the Regional Director
  2. Record Section/Office of the Regional Director will:
    • Track/Barcode
    • Advise Applicants to proceed to Cash Section for the Documentary Stamp payment
    • Forward application to Legal Unit
  3. Legal Unit will:
    • Validate and process
    • Issue Resolution for the approval of the Correction of Name/Date of Birth
    • Forward the Resolution to RD’s Office for approval and signature of RD
  4. Regional Director’s Office will:
    • Approve and Forward the document/s to Records Section
  5. Records Section will:
    • Release the Approved Document/s to Client/Applicant



Equivalent Record Form(ERF), Reclassification Of Position And Conversion
REQUIREMENTS

PROCEDURE
  1. Client/Applicant will submit application for ERF, Reclassification and Conversion of Position document/s to Records Section/Office of the Regional Director
  2. Record Section/Office of the Regional Director will:
    • Track/Barcode
    • Forward ERF, Reclassification of Position document/s to Personnel Section
  3. Personnel Section will:
    • Validate and Process
    • Prepare Endorsement for DBM
    • Forward to Regional Director’s Office for signature
  4. Regional Director’s Office will:
    • Sign and Forward the Endorsement for ERF. Reclassification of Position and Conversion of Position to Personnel Section
  5. Personnel Section will:
    • Segregate according to Position and Division
    • Forward the Approved ERF, Reclassification of Position and Conversion of Position document/s to the Department of Budget (DBM) for the Issuance of Notice of Organization, Staffing and Compensation Action (NOSCA)
  6. Records Section (after DBM’s approval of NOSCA) will:
    • Pick-up the NOSCA
    • Submit the NOSCA to RD Office for notification
    • Forward the NOSCA to Budget and Finance Division
  7. Budget and Finance Division will:
    • Receive the NOSCA
    • Prepare Endorsement to the Schools Division Offices concerned



GSIS Retirement/Survivorship/Separation/Total And Permanent Disability Claim Application
REQUIREMENTS
  • Endorsement from the Division Office
  • Application for the retirement/Survivorship/Total and Permanent Disability/Separation
  • Updated Service Record
  • Clearance from Division Office pertaining to money and property accountability
  • Certification that the employee has no pending Administrative Case and or Criminal Case filed against him/her from the Division Office
  • Certificate of Last Payment from Regional Payroll Services Unit
  • Provident Fund Loan Clearance from the Division Office
  • GSIS Tentative Computation
PROCEDURE
  1. Client/Applicant will submit the claim application to Records Section/Office of the Regional Director
  2. Record Section/Office of the Regional Director will:

    • Forward the claim application to Personnel Section
  3. Personnel Section will:

    • Prepare Endorsement for GSIS

Authority To Travel Abroad Procedure
REQUIREMENTS
  • Approved Form 6
  • Division Clearance
  • Certification as to the following:
    1. Another teacher is available to take over the classes;
    2. Teacher has no pending unsubmitted report;
    3. Teacher has no pending administrative case;
    4. No government fund will be spent for the travel;
    5. Teacher has not gone on leave abroad within one year prior to the date of his/her application
PROCEDURE
  1. Client/Applicant will submit the Authority to Travel Abroad Application to Records Section/Office of the Regional Director
  2. Record Section/Office of the Regional Director will:
    • Track/Barcode
    • Forward the application to Personnel Section
  3. Personnel Section will:
    • Validate and process
    • Prepare Endorsement for the signature of the Regional Director
    • Forward to Regional Director’s Office
  4. Regional Director’s Office will approve and Forward the Authority to travel to Record Section
  5. Records Section will:
    • Release to Division Office



Terminal Pay Application
REQUIREMENTS

    • Latest Salary Adjustment for Non-Teaching Position
    • R.A. 4670 for Teaching Position
    • R.A. 7305 Magna Carta of Public Health Workers (PHWs) for compulsory retirement
  • Last Approved Appointment
  • Certification of Last Salary Received stating the gross amount, date and monthly salary rate from the Division Office
  • Clearance from Division Office pertaining to money and property accountability
  • Affidavit of Applicant that he/she has no pending criminal/administrative case (R.A. 3019) notarized by the Lawyer or Mayor
  • Certification that the employee has no pending Administrative Case and or Criminal Case filed against him/her from the Division Office
  • Fiscal’s Clearance from Place of Assignment and Residence
  • GSIS Clearance
  • GSIS Retirement Adjudication of Annuity Voucher
  • Sworn Statement of Assets and Liabilities and Net Worth, latest
  • Certificate of Last Payment from Regional Payroll Services Unit
  • DBP Passbook with clear Account Number
  • Provident Fund Loan Clearance from the Division Office
  • In case of deceased claimant, the following should be submitted:
    1. Original/Certified true copy of Birth Certificate of all Children of deceased
    2. Marriage Contract
    3. Death Certificate
    4. Proof of Surviving Legal Heirs
  • Affidavit of relationship executed by the next kin’s ages of children should be stated
PROCEDURE

  2. Record Section/Office of the Regional Director will:

    • Forward the claim application to Personnel Section
  3. Personnel Section will:

    • Compute commutation of vacation/sick leave credits
    • Prepare Memorandum for payment of the Schools Divisions for verification of pending and no pending case
    • Forward to Legal Unit
  4. Legal Unit will:
    • Receive
    • Verify (Pending/No Pending Case)
    • Forward the claim application to Finance Division
  5. Finance Division will:
    • Prepare endorsement
    • Forward to Regional Director’s Office
  6. Regional Director’s Office will approve and forward the claim application to Finance Division
  7. Finance Division will segregate and forward to Records Section
  8. Records Section will release to Division Office
